1. When did you first notice the problem, and when does it occur?
2. What does it feel like, and what makes it worse or better?
3. Where did the problem start, and has it shifted location over time?
4. Why, exactly, are you at the doctor’s office? (For example, maybe you want to know if you need to take antibiotics, change your diet, or cancel your vacation.)
